Harken 5mm Stainless Steel U-Adaptor
The 093 Harken metal U-adaptor allows blocks with 3/8 in (10 mm) posts to be attached to swivel bases or to other blocks with 3/8 in (10 mm) posts.
Simply put the two pins though the two blocks and they'll be linked up. See the product photos for an example.
